In 2012, more than 330,000 women in the United States elected to have breast augmentation surgery; worldwide, that number exceeded 1.5 million. Although media coverage suggests otherwise, only a minute percentage of the women who get breast implants are actresses and models. They come from all walks of life; they include policewomen and CEOs, teachers and soldiers, young mothers and nurses.
About 30 percent of the women who get breast implants are in their 20s. They often report being self-conscious about their lack of development or embarrassed about their breasts being asymmetrical or uneven. About 35 percent are women in their thirties, many of whom lost breast volume after childbirth. Some want to recapture their pre-pregnancy breast size, while others liked the breast fullness they had during pregnancy and want to recreate it with implants.
Whether women are having cosmetic breast surgery (augmentation, lift or reduction) or reconstructive surgery after a mastectomy, their goals are similar: They want to look feminine, natural and proportional. They want their clothes to fit better.
In consultation, women ask dozens of questions about the safety, durability and longevity of saline and silicone gel implants – the two types that are approved for sale by the U.S. Food and Drug Administration.

Breast augmentation surgery may be a surefire way to get the breasts you ultimately desire, but when it comes to making decisions, there are a variety of factors to take into consideration. A properly trained plastic surgeon should be able to guide you through your decision-making process. One of those decisions will be if your implants should go over or under the muscle. So, what’ the difference?
Subglandular: Over the Muscle
The upside: Since the implant is placed over the pectoral muscle and below the breast tissue, recovery time is usually shorter because there is fewer traumas to the underlying tissues. Placing the implant over the muscle can also give a slightly lifted look.
The downside: The implant sits closer to the surface of the skin, making it more detectable through touch and more visible if your skin is thin. In a mammogram, additional views may be necessary.
Silicone implants allow for more patients to have their implant placed subglandularly (in front of the muscle) because there is less rippling and visibility with silicone.

Submuscular: Under The Muscle
The upside: Positioning the implant below both the pectoral muscle and the breast tissue allows for the implant to be less visible and act as an internal bra, keeping the implant in place. The implant is also only partially covered by muscle, so it’s less likely to cause problems with mammograms.
The downside: It’s a more painful recovery because the internal anatomy of the tissue behind the breast is hanged more.
Placing the implant under the muscle is good for those who are thin because it provides extra coverage over the implant, which gives a more natural look.
implant placement
Breast augmentation helps women with small or unevenly-sized breasts achieve a fuller, firmer and better-proportioned look through the placement of breast implants. Women may elect to undergo this procedure for many different reasons, including balancing breast size and compensating for reduced breast mass after pregnancy or surgery.
During your breast augmentation exam, Dr. Bishara will examine your breasts and perhaps take photographs for your medical record. Dr. Bishara will then examine the size and shape of your breasts for Breast augmentation surgery, the quality of your skin and the placement of your nipple and areola. Also, a breast lift may be recommended with the breast augmentation.  Please call our office at (817) 473-2120 or click on the link below.  You can also visit us on our website at www.MarkBisharaMD.com and our Facebook page at Paragon Plastic Surgery and Med Spa at www.facebook.com/pages/Paragon-Plastic-Surgery-and-Medspa

Plastic surgery is one of the most common procedures performed in the United States and the transformative power compels thousands of men and women to go under the knife every year. Unlike most surgeries, cosmetic procedures are not medically necessary which can make it hard to analyze the demographics. While each individual’s needs and wants differ when it comes to choosing a procedure, the most popular cosmetic surgeries can be broken down by age group. While trends in plastic surgery vary from year to year, the American Society of Plastic Surgeons 2012 statistics report shows that some procedures are consistently popular with certain age groups. Here’s why.

Nose reshaping is widespread among 13-19 year-old patients, simply because the nose you were born with is the nose you will have for life. While other parts of the body can change given time or lifestyle habits, the shape and structure of your nose changes the least. The skin does thicken and the cartilage can lose its support, but for the most part, the defining features remain constant throughout life. Adolescents who are approaching adulthood oftentimes turn to rhinoplasty to get a new start or a fresh look as they embark on milestones such as high school graduation or the start of college.
On the other hand, breast augmentation is a popular procedure for those who have gone through puberty: 20-29 year-olds. The breasts are typically fully formed by that age, and the size and shape are at a fixed point. Shifts in weight, pregnancy and lifestyle (bra usage) may alter breast size and shape but in an unpredictable way. This is a time when image is very important to women, so they seek larger breasts in order to boost their self-confidence and balance the proportions of their new adult figures.
Once men and women reach their 30’s, their metabolism begins to slow down. They notice a change in the way their bodies deposit fat even if their exercise pattern and diet have remained the same. Certain body parts store stubborn fat that is hard to lose even with healthy eating and exercise. Liposuction has traditionally been the remedy to this problem. We have seen an increase in the popularity of newer technologies that provide less invasive options for fat reduction. However, liposuction remains time honored and consistent in providing the most predictable results. A variety of techniques are available to safely remove fat and sculpt curves.
The 40’s are a time when the aging process really begins to set in. We typically start to see the earlier signs of aging around the eyes and forehead. The skin around the eye is prone to wrinkle and show redundancy. The upper eyelids and forehead can begin to droop and give a tired or sad appearance. Thus it’s no surprise that patients between the ages of 40-54 are likely to undergo an eyelid and/or brow lifting surgery in order to minimize these changes. The lower eyelids are treated by removing or tightening skin as well as treating fat pockets.
Finally, the facelift is an extremely popular procedure for those 55 and older. At this point, there are multiple parts of the face that need rejuvenating. The cheeks typically fall or deflate, the corners of the mouth turn down, jowls appear, the jawline loses its contour and the neck becomes loose and droopy at times, showing bands under the chin. A facelift helps restore a youthful appearance by addressing these parts that have aged including the eyelids, sagging of the neck and volume loss in the cheeks. Combinations of procedures can lift the brow, the mid face or cheek area and tighten the jaw line while treating jowls and reshaping the neck.
Of course everyone ages differently and each person’s goals are different, but these tend to be the most popular procedures by age group. Do any of these surprise you? What procedure have you been thinking about?
Dr. Mark Bishara provides a wide range of cosmetic procedures to help patients look and feel their best. These procedures are designed to improve the appearance of the face and body through minimally invasive techniques that provide highly effective results. Augmentation mammoplasty, or breast augmentation is a surgical procedure to increase the size, shape or fullness of a woman’s breasts. The surgeon places silicone, saline or alternative composite breast implants under the chest muscles or breast tissue.
Breast augmentation is done to:

  • Enlarge breasts that are naturally small.
  • Restore breast size and shape after pregnancy, weight loss or breastfeeding.
  • Restore symmetry when the breasts are asymmetrical
  • Restore the breast(s) after surgery as treatment for breast cancer or some other condition or event that affected the size and shape of the breast.

Breast enlargement can boost self esteem- women usually experience a significant boost in self-esteem and positive feelings about their sexuality after undergoing breast enlargement.
98% of women who underwent breast augmentation surgery said the results met their expectations. Women also reported improvements in quality of life and self-esteem after their procedure.
A study, involving 225 women who were followed for five years, found that:

  • Patients said they were “back to normal” within 25 days of their surgery
  • 85% said their new breast size was “just right”
  • 13% said a larger size would have been better
  • Less than 2% would have preferred a smaller size
  • 1% were not happy with their scars, which are usually in the crease under the breast
  • 75% thought their breast firmness was “just right”
  • 98% said their results met with their expectations

What are breast implants?

A breast implant is a medical prosthesis that is placed inside the breast to augment, reconstruct or create the physical form of the breast.
The outer layer of breast implants is made of firm silicone. The inside of the implant is filled either with salt water (saline solution), silicone gel, or a composite of alternative substances. Makers of breast implants say they should last for at least ten years.

Saline breast Implant
Breast implants filled with saline solution

There are three main types of breast implants:

    • Saline implants – filled with a sterile saline solution (biological-concentration salt water 0.90% w/v of NaCl, ca. 300 mOsm/L.) which is held within an elastomer silicone shell. These implants can be filled with different amounts of saline solution, which may affect the feel, firmness and shape of the breast. If this type of implant leaks, the solution will be absorbed and expelled by the body naturally.

    • Silicone implants – filled with a viscous silicone gel which is held within an elastomer silicone shell. If a silicone-filled implant leaks, the gel will either stay in the shell or escape into the breast implant pocket. A leaking silicone-filled implant might not collapse. Patients choosing this type of implant should carry out more regular checks with their doctor compared to those on saline solution implants. The condition of the implants can be determined with an MRI or ultrasound scan.

What happens during the breast augmentation procedure?

Anesthesia

The patient may have a choice of general or local anesthesia. Under local anesthesia the patient is awake and only the surgical area is numbed. However, in most cases the procedure is done under general anesthesia.

The incision

The surgeon and patient should discuss incision options. The doctor should explain which are most suitable for the patient and her desired outcome.
The following incision options are possible:

  • Inframmammary incision – in the crease under the breast.
  • Transaxillary incision – in the armpit (axilla)
  • Periareolar incision – around the nipple

The choice of which incision to use depends on several factors, including the degree of enlargement, the patient’s anatomy, the type of implant, and surgeon-patient preference.

Inserting and placing the breast implant

Breast Implants
1. Breast after subglandular augmentation.
2. Breast after submuscular augmentation.

The breast implant is inserted into a pocket. Today, there is a choice between two types of placements:

  • A submuscular placement – under the pectoral muscle. When placed here the patient may take slightly longer to recover, compared to subglandular placement, and may also experience a little more post-operative pain.
  • A submammary (subglandular) placement – behind the breast tissue, over the pectoral muscle.

Techniques used today for placing breast implants have significantly improved recovery times.

The incisions are closed

The surgeon closes the incisions with layered sutures (stitches) in the breast tissue, and with skin adhesives, sutures and surgical tape to close the skin and keep it closed.
Initially, there will be evident incision lines. These will fade with time.

Breast augmentation plastic surgery is one of the most common procedures performed annually by members of the American Society for Aesthetic Plastic Surgery. Women may choose to under go breast enlargement surgery for various reasons. These personal reasons may center around breasts that are perceived to be under developed, or because of differences in the sizes of the breasts or from changes after pregnancy or breast feeding.
Some women may be happy with their breasts but just want them made fuller. Often after weight loss, aging or childbirth a woman’s breast volume and shape may change. This too can lead to a woman to seek a breast augmentation. Breast implant surgery performed by cosmetic plastic surgeons is the most popular way to improve breast shape and size. Breast enhancement using breast implants can give a woman more proportional shape and may improve self esteem.
Before and After Pictures of Dr. Mark Bishara’s Patients:
Are you a good candidate for breast augmentation?
One or more of the following feelings or conditions may indicate that you are a good candidate for breast augmentation:

  • you are bothered by the feeling that your breasts are too small
  • clothes that fit well around your hips are often too large at the bustline
  • you feel self-conscious wearing a swimsuit or form-fitting top
  • your breasts have become smaller and lost firmness after having children
  • weight loss has changed the size and shape of your breasts
  • one of your breasts is noticeably smaller than the otherDuring the consultation, you will be asked about your desired breast size and anything else related to the appearance of your breasts that you feel is important. This will help your surgeon to understand your expectations and determine whether they realistically can be achieved. How will my plastic surgeon evaluate me for breast augmentation surgery?Your plastic surgeon will examine your breasts and take photographs for your medical record. He or she will consider such factors as the size and shape of your breasts, the quality of your skin and the placement of your nipples and areolas (the pigmented skin surrounding the nipples). If your breasts are sagging, a breast lift may be recommended in conjunction with augmentation.
You should come to the consultation prepared to discuss your medical history. This will include information about any medical conditions, drug allergies, medical treatments you have received, previous surgeries including breast biopsies, and medications that you currently take. You will be asked whether you have a family history of breast cancer and about results of any mammograms. It is important for you to provide complete information.
If you are planning to lose a significant amount of weight, be sure to tell your plastic surgeon. He or she may recommend that you stabilize your weight prior to undergoing surgery. If you think that you may want to become pregnant in the future, you should mention this to your surgeon. Pregnancy can alter breast size in an unpredictable way and could affect the long-term results of your breast augmentation. There is no evidence that breast implants will affect pregnancy or your ability to breast-feed, but if you have questions about these matters, you should ask your plastic surgeon.

What type of breast implants can be used for breast augmentation?

breastaug c2The implant is placed in a pocket either directly behind the breast tissue (right) or underneath the pectoral muscle which is located between the breast tissue and chest wall.

The size and type of breast implant recommended for you will be determined by your goals for breast enhancement, your existing body frame, and mass, your existing breast tissue, and the preferences you and your plastic surgeon discuss.  All breast implants include a solid silicone rubber outer shell, called a lumen.
    Breast implant options include:

    • Saline breast implants: Filled with sterile salt water
    • Silicone filled breast implants: Filled with soft, elastic gel

New scientific data on the safety of breast implants is rapidly being collected. In the future, it is possible that additional types of filler materials may become available as advancements take place. Surgical techniques for breast augmentation and breast implants themselves are continuously being refined, increasing the safety and reliability of the procedure. Your plastic surgeon will be able to provide you with the latest information.

Maintaining a Relationship with your Plastic Surgeon

Please remember that the relationship with your plastic surgeon does not end when you leave the operating room. If you have questions or concerns during your recovery, or need additional information at a later time, you should contact your surgeon.
