Dr. Mark Bishara would like to reward patients who refer their friends for cosmetic or robotic hair procedures with credit toward a future Skin Clinic procedure. Following are the guidelines for our Paragon Perks Program:
- We will credit patient $50 in services or each patient that schedules a consult appointment.
- We will credit patient $100 in services for each patient that schedules a cosmetic or robotic hair restoration surgery (value of $2,000 or more)
- All credits can be applied to a single full price laser procedure or injectable
- The referred patient must provide the referrals full name when scheduling their consultation appointment.
- The credit can not be combined with any other discount
- They must complete their procedure within 6 months of the initial consultation.
- Credits can only be used for future treatments; they cannot be applied as a payment on an outstanding balance from a previous treatment.
- Referral credits are transferable to friends and family

- Laser 360/Paragon Liquid Laser Lift
- Laser Hair Reduction
- Laser Tattoo Removal
- IPL/Laser Photofacial
- Laser Skin Tightening
- Fillers & Injectables such as Botox, Radiesse and Juvederm
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
